PFBH is a proponent of patients’ self-advocacy and is committed to providing fundamental educational tools for understanding their psychiatric conditions and treatment options.
Our Forms
Our intake forms and HIPAA paperwork are available for perusal and completion. Existing patients may simply complete the Release of Information Form to authorize us to release to or obtain information from any of their other providers, simplifying coordination of care.
While PFBH does not handle emergent psychiatric concerns, we feel it is of the utmost importance that patients know what resources are at their disposal, should a crisis arise.
