Bring your PDH insurance card. It includes important info we need like your name, member ID, group number, employer name, and billing details. This helps us verify your coverage and get you seen faster.
We submit your claim directly to PDH using standard coding and the instructions on your ID card.
You’ll only be responsible for your copay, deductible, or co-insurance as outlined by your plan.
Your Explanation of Benefits (EOB) will show what’s covered and what’s been paid.Claims are submitted to the address or Payor ID listed on your PDH insurance card. We’ll handle that part for you.
